Income Tax Assistance with AARP Volunteers Thursday, Feb 2 – April 7, 9am–3pm Trained volunteers will assist in the completion of state and federal income tax forms. The volunteers do not assist with city tax forms. Bring these items: Copy of last year’s income tax return (s) and supporting documentation W-2 & W-2p forms from each employer if you earned a salary; SSA-1099 forms (1099-INT, 1099-DIV, 1099-MISC, etc) showing interest, and or dividends and original purchase price of sold assets 1099 form from the payer of your pension or annuity Unemployment compensation statements All forms indicating federal income tax paid All receipts or cancelled checks if itemizing deductions Your social security card. Winter Class Registration, Refund and Guidelines and Fitness WINTER CLASSES - Registration Refund Guidelines Senior Center membership is required to participate in Senior Center activities including classes, activities and programs. Registration for winter classes began on Monday, November 28 FOR ALL CLASSES. You may register for one other person only. If you don't come to in-person registration, please register at least one week before the beginning date of each class. The City of Mentor accepts Visa and Mastercard. When registering online, Discover and American Express may also be used. PROGRAM/ACTIVITY REFUND GUIDELINES PLEASE READ CAREFULLY. 1. Full refund or transfer if Mentor Senior Center cancels the program/activity. 2. An administrative charge of $10 will be assessed if the participant cancels out of the program/activity. Prevent the $10 fee by transferring the full amount to another program/activity within the same season. Students assume risk of change in personal affairs and health. 3. Transfers can be made with full credit, up to the first day of the program. Transfers must be within the same season and year of programs (spring to spring, or fall to fall) and into a program that has available space. The application deadline for spring classes is January 4. Please get an application at the desk or on the rack at the Munson Road entrance. Guests Guests are welcome to participate in programs that do not require registration. Guests or those interested in membership may register for one program or 2 trips. We ask that anyone participating at the center more than once or twice a year join as a member. Unfortunately guests are not able to use the fitness room. Sample Classes All seniors are welcome to take a class one time before registering for the program to ensure it is the right fit.
