Peritoneal Dialysis Goes Live with the eKHR The PD Units at Seven Oaks General Hospital and St. Boniface Hospital introduced the Patient Registration/ Demographics and Medication Modules in May 2014. Thanks to the nurses, clerks and pharmacists for their help with inputting their patients’ data.

Medication Flow Sheet After much collaboration this form is now in the final stages of development and will be rolled out in the coming months. Stay tuned for further updates.

Medications Prior to Admission – Reconciliation and Order Form The Medications Prior to Admission – Reconciliation and Order Form has been approved and will replace the Hospital Admissions form. This form, generated from the eKHR, is a tool used by staff (e.g. Emergency) to facilitate medication reconciliation at the time of admission. Plans for training other departments in its use are underway.

Dialysis/Vascular Access Data Entry at HSC In June 2014 Kristen Csupak and Younghee Lee began the monumental task of entering the vascular access history for all HSC’s patients. They have completed data entry for patients in CDU, SCDU, Local Renal Health Centres and those Renal Health patients that have had had vascular assessments. Congratulations on your accomplishments for a job well done!

Dialysis Access Reports Dialysis (Vascular) Access Review Reports, generated from the eKHR, are being added to patients’ renal health records. Two reports will be available for each patient:  Current Access, Detailed View – the details regarding the current access, including assessments, investigations, procedures and access status.  All Accesses, Summary View – an overview of current & previous accesses, including access type, body side, location, date created & current access status.

Quick Tips When you make or discover an error in medication entry in the eKHR remember to CANCEL THE MEDICATION. If you only discontinue the incorrect entry the error will print on the new Medication Flow Sheet as a discontinued medication. Consequently it appears as if it were actually a medication order rather than a data entry error. Cancelled medications will not print on the Medication Flow Sheet. Updating the Dialysis Treatment Status is very important so everyone knows where a patient is at any given time. When   

Get to Know Team Merit Alison Lindsay Ally is the MRP’s Transition Coordinator plus provides clinical and education support to the MeRIT Team. Outside of work she is a busy wife and mom for 2 children and 2 dogs. She enjoys physical fitness, cooking, reading and spending time at the cottage. Soccer is a “huge” part of the Lindsay family’s life.

Changes to Combination Dosages You can now enter scheduled and PRN doses together as one combination dose rather than two separate entries. PRN medications will still appear under the PRN section of the Patient Medication Card.

Cognos Boards Please remember that while the eKHR and Cognos Display Boards are linked, they are two separate entities. When the Cognos Boards go down the eKHR should still be available for staff to use.

Demographic Report

The MRP Policy & Procedure Committee is revising the Renal Patient Kardex. As part of this project the Demographic Report will soon be available through the eKHR. It will include: Patient name, PHIN, MRN, DOB, ACP Goals of Care, Allergies, Photo, Address, Emergency Contact, Program, Dialysis Type & Centre.

Training The 2015 Schedule for Module Orientation has been distributed to all sites. Please note that that training will now be extended to two full days. Please remember that managers are required to register employees for these sessions with Sharon Edmundson.
