DONORSCHOOSE.ORG ( Contract #07891): First 5 LA has partners with DonorsChoose.org, an online charity, to provide funding to public school, preschool, and transition kindergarten classrooms in Los Angeles County. During each year of this 4-year project, First 5 LA will match donations, dollar for dollar, to eligible DonorsChoose.org classroom requests up to $112,500 per year for the total project - leveraging up to $225,000. DonorsChoose.org has met their goals during the first 2 years of this project.

CALIFORNIA COMMUNITY FOUNDATION COMMUNITY INVESTMENT FUND (CCFCIF) (Contract #08022): CCFCIF is the fiscal sponsor for the LA Partnership for Early Childhood Investment (The Partnership), a philanthropic collaborative comprised of public/private investors in early childhood. The Partnership will establish and manage a pooled fund to LA Partnership for Early support children prenatal to age 5 in Los Angeles County. Five hundred thousand dollars is dedicated to support program operations and the remaining $1 million will Childhood Investment be used to create the pooled fund, called the Baby Futures Fund. The $1 million will only be awarded if the Partnership is able to secure other matching funds and will be distributed as each match is secured. During the first year of this project, the fund was established. During the second year, criteria was established for the fund and initial projects were funded. It is expected that during the third year, they will be able to continue to be on target for their match requirements.

FAMILIES IN SCHOOLS (Contract #08131): First 5 LA has partnered with Families In Schools (FIS) to serve as Program Administrator for the Challenge Grants and Social Enterprise Grants Programs. The project called for Challenge Grants (CG) to be awarded to 7-11 grantees at a rate of Challenge Grants/Social 1:1 over a 2-year period as grantees secure new funds from non-First 5 LA sources and the Social Enterprise (SE) Grants to provide 5-7 grantees with capital necessary Enterprise Grants to launch or expand an earned income strategy. In addition to awarding $1.4 million Program - Program in grant funds, FIS will continue to provide CG and SE grantees training and Administrator technical assistance support. During the first year, both the CG and SE programs were developed. In the second year, both programs selected grantees and started implementation. Currently the CG grantees are on target to meet their match fund requirement. The SE grantees will continue to develop and implement their projects in year three.

Healthy Food Access Initiative- Children's Garden Collaborative

LOS ANGELES CONSERVATION CORPS (Contract #08182): The Children’s Garden Collaborative will begin year two of its five year award to build community gardens throughout LA County. In FY 12-13, the LA Conservation Corp and its collaborative partners, successfully opened two community gardens, providing access to 63 gardening beds of which 90% are currently in use and trained families on gardening and nutrition in a series of classes. During FY 13-14, the collaborative plans to complete construction of three community gardens in LA County to improve access to healthy foods for children 0-5.

AMIGOS DE LOS RIOS (Contract #08200): In FY 12-13, Amigos De Los Rios completed two (2) parks in the city of Maywood: Riverfront Park and Pixley Park. During FY 13-14, Contractor expects to implement three (3) Tot Park and Trail projects in the following play spaces: Los Angeles County Wellness Center Park, Salazar Park and City Terrace Park.

THE CHILDREN'S CLINIC, SERVING CHILDREN AND THEIR FAMILIES (Grant #07861): The Healthy Smiles Pilot Program trains pediatric providers to perform oral health screenings as part of each pediatric examination; conduct assessment of the child's risk Oral Health and Nutrition for disease and make referrals for more extensive dental care when appropriate. Medical Expansion and Enhancement assistants are trained to provide fluoride varnishing when appropriate and community Project health workers provide on and off-site assistance with oral health education. By the end of the project (June 30, 2014), they will provide at least 1200 unduplicated children ages 0-5 with preventive oral health treatments.

SOUTH BAY CHILDREN'S HEALTH CENTER (Grant #07882): The South Bay Oral Health Project (0-5 focus) will expand their comprehensive dental Oral Health and Nutrition care for children, focusing on children ages 0 to 5 in pre-K and Kindergarten programs in Expansion and Enhancement Wilmington, Gardena, Carson, Harbor City and Lomita who are not currently being served Project by SBCHC within these cities. By the end of the project, they will provide at least 3600 unduplicated children ages 0-5 with preventive oral health treatments.

WESTERN UNIVERSITY OF HEALTH SCIENCES (Grant #08026): The First 5 LA OHN Project will expand access to dental services to the 0-5 population at Oral Health and Nutrition WIC sites in El Monte; Head Start programs in the Mountain View School District and El Expansion and Enhancement Monte School District; San Gabriel Pomona Valley Regional Center and Kindergarten Project children enrolled in the El Monte Unified School District. By the end of the project, they will provide at least 14,827 unduplicated children ages 0-5 with preventive oral health treatments.

UNIVERSITY OF THE PACIFIC (Grant #08219): The First 5 LA Virtual Dental Home Demonstration Project will bring the Virtual Dental Home (VDH) model to underserved communities with children 0-5 years of age who lack access to oral health care and oral health education in SPA 5. Through this model and partnership with Venice Family Clinic, a registered dental hygienist in alternative practice and a dental assistant/navigator will deliver Virtual Dental Home services through telehealth technology in selected community sites in SPA 5 to children ages 0-5 who do not currently have a dental home.

UNIVERSITY OF SOUTHERN CALIFORNIA (Grant #08218): The Ostrow School of Dentistry of USC's Children's Health And Maintenance Project (CHAMP) will utilize a case management model to address access to oral care for Oral Health and Nutrition underserved children ages 0-5. After the CHAMP team (faculty/students/staff) completes Expansion and Enhancement an oral exam and preventive care at a community site (WIC, Head Start, etc.), interns from Project - Dental Health the USC School of Social Work, an oral health promotora and a Benefits Enrollment Innovation Specialist will be available to provide referral, enrollment and follow up case management to link them to a dental home. Portable dental equipment will be used for fluoride treatments at the community sites, but where space is a premium, the USC Mobile Dental Van will be utilized.

BIENVENIDOS CHILDREN'S CENTER (Grant #00682) Provides culturally appropriate in-home case management and counseling services to two targeted populations: families with children 0-5 and pregnant women, both determined to be at risk for child maltreatment residing in SPA 7. Grantee projects to enroll 51 new eligible families referred by DCFS and 29 new pregnant women into case management and counseling services. In addition, grantee projects to provide $35,000 in concrete and financial supports to program participants and education, group and in-home training supports to 260 unduplicated individuals residing in SPA 7. Capacity building services regarding best practices in addressing child abuse and neglect will include 348 staff training hours and 24 collaborative meetings.

PARA LOS NINOS (Grant #00685) Provides culturally appropriate family strengthening services including in-home case management and counseling to families with a child 0-5, referred by DCFS and pregnant women referred by community-based organizations at high risk for child-maltreatment residing in SPA (4); and in addition provides education, group supports and capacity building services to those residing and working in SPA (4) regarding best practices in addressing child abuse and neglect. High risk is determined for pregnant women through either a history or current experience with one or more of the following risk factors: domestic violence, substance abuse, maternal depression/ mental illness, and young maternal age.

ST. JOHN'S CHILD & FAMILY DEVELOPMENT CENTER (Grant #00686): Provides culturally appropriate in-home case management and therapy services to two targeted populations: families with children 0-5 and pregnant women, both determined to be at risk for child maltreatment residing in SPA 5. Grantee projects to enroll 20 new eligible families referred by DCFS and 19 new pregnant women into case management and therapy services. In addition, grantee projects to provide $20,250 in concrete and financial supports to program participants and education, group and in-home training supports to 33 unduplicated individuals residing in SPA 5. In addition, grantee projects to provide $23,016 in concrete and financial supports to program participants and culturally based interactive education, group and inhome training supports to 341 American Indian adults and children residing in Los Angeles County. Capacity building services regarding best practices in addressing child abuse and neglect will include 828 staff training hours and 12 collaborative meetings.

SPIRITT FAMILY SERVICES (Grant #00688): Provides culturally appropriate in-home case management and counseling services to two targeted populations: families with children 0-5 and pregnant women, both determined to be at risk for child maltreatment residing in SPA 3. Grantee projects to enroll 31 new eligible families referred by DCFS and 18 new pregnant women into case management and therapy services. In addition, grantee projects to provide $50,000 in concrete and financial supports to program participants and education, group and in-home training supports to 75 unduplicated individuals residing in SPA 3. Capacity building services regarding best practices in addressing child abuse and neglect will include 492 staff training hours and 16 collaborative meetings.

SHIELDS FOR FAMILIES (Grant #00687): Provides culturally appropriate in-home case management and counseling services to two targeted populations: families with children 0-5 and pregnant women, both determined to be at risk for child maltreatment residing in SPA 6. Grantee projects to enroll 48 new eligible families referred by DCFS and 20 new pregnant women into case management and counseling services. In addition, grantee projects to provide $8,000 in concrete and financial supports to program participants and education, group and in-home training supports to 100 unduplicated individuals residing in SPA 6.
