Clinical Tools to Consider Pre-Standing Safety check
When Do I use It? The Pre‐Standing Safety checklist is designed to encourage consistent use of musculoskeletal safety principles when assisting with patient transfers and mobility. This checklist should be completed before manual transfer and/or mobilizing.
Schmid Screening Tool
The Schmid screening tool is used when the pre-hospital screening tool has identified a patient at risk for falls. This tool will help identify the level of risk. This is an assessment tool used to identify and evaluate patients at risk for falls. All patients receiving care in a VIHA facility and care setting will be screened for risk of falls.
Morse Fall Scale
The Morse Fall Scale is used when the pre-hospital screening tool has identified a patient at risk for falls. This screening tool attempts to identify those at highest risk of falling and to help identify which interventions to implement for the individual patient. All patients receiving care in a VIHA facility and care setting will be screened for risk of falls.
Humpty Dumpty Scale
The Humpty Dumpty Scale is used on all pediatric patients receiving care in all VIHA facilities and care settings to screen for risk of falls, and appropriate falls prevention strategies will be implemented.
