INSTRUCTIONS: Use ALL college credits ever taken anywhere for which you were given a grade. Multiply the number of credits by the points for that grade. Add the column of credits, then add the quality points. Divide the quality points by the total of credits for each cumulative average. For a Committee Interview, this form should include your most recent grades from the fall semester, as well as ALL previous college credits.
